Completed Bright From the Start PreK Registration Form (print on website [below] or pick up at BPS) | |
Completed Berrien County Enrollment Form (print on website or pick up at BPS) | |
Proof of Residence (lease/rental agreement, electric, water, or gas bill with street address indicated and in the parent/guardian's name) | |
Current and Complete Immunization Certificate (DHR Form 3231 or Appointment Card) | |
Current and Complete Hearing, Vision, Dental, and Nutrition Screening (DHR Form 3300 or Appt. Card) | |
Certified copy of the child’s birth certificate. |
** Student MUST be registered by his/her primary custodial parent (in cases of divorce) or legal guardian. **
Age Requirements:
Children must be 4 years of age on September 1, 2026
Children who are 5 on September 1, 2026 AND have not attended Georgia's PreK Program as a 4 year old may be eligible as well.
